Course Title:   Aircraft Accident Investigators Course - Pilot Training

A comprehensive course that provides the Flight Safety Officer with all the tools to effectively investigate, analyse and report on aircraft accidents and incidents. Based on international standards and presented by the top subject matter experts in South Africa. Who should attend: Any pilot, air traffic controller, aviation maintenance engineer or aviation operations personnel who have completed a recognised flight safety officers course or proof of similar qualification (proof of qualification required before acceptance).

Modules covered:

  • Introduction, Statistics & Causes of Accidents.
  • Conducting Preliminary Investigations, Securing the site, Gathering the evidence and dealing with authorities.
  • Recording the evidence: Aircraft Recorders, ATC Records, Computers & Simulators.
  • Witness Interviewing.
  • Convening the Investigation Board and Selection of Members.
  • Wreckage Distribution, Impact Forces & Angles and accident diagrams.
  • Accident Photography.
  • Fire Investigation.
  • Aerodynamics: Fixed wing, High Speed & Helicopters.
  • Structural Investigation: Propellers, Airframes, Engines and Controls, Loads Stresses & Strains, and Structural Failure.
  • Investigating Maintenance related causes.
  • Turbine Engine Investigation.
  • Aircraft Systems: Avionics, Instruments, Cables and Rods, Wiring, Fuel systems, Hydraulic Systems & Oil and coolants.
  • Tires & Runway Accidents.
  • Weather: Ice, Rain, Turbulence & Microbursts.
  • Mid Air Collisions.
  • Operations & Maintenance.
  • Airfield & ATC.
  • Navigation Systems.
  • Investigating for Human Factors: Causes, Error Management and Investigation Models, Human Factors Accident Classification System.
  • Aviation Medicine and Forensic Pathology.
  • Aircraft Performance: Stalls & Spins, Down-wash & Wing Tip Vortices, Loss of Performance, Ground Operation Hazards, Stability & Control.
  • Crash Survivability.
  • Analytical Techniques
  • Report Writing based on ICAO Annex 13.
  • Investigation Management: Dealing with authorities (national and international).

Practical Phase:
Students will be required to investigate and report on two aircraft accidents towards the end of the course so as to provide practical experience and knowledge on the process and methods of accident investigation.

Package:
Students receive a comprehensive working manual, 5 soft cover books (internationally recognised) on accident investigation methods and analysis, soft carry-briefcase, stationery, and CD’s with analytical techniques and methods and investigative data.

Meals and Refreshments:

Costs cover teas, snacks and daily lunch.
